Case of Osteosarcoma of THIGH BONE

Presentation

  • 18-year-old male
  • Swelling and pain in right knee

Examination

  • Tenderness right distal femur
  • Lump arising from deep structure
  • Knee ROM full
  • Functional Pain present (pain on weight bearing increases)
Pre Op Xrays – Lesion seen in distal femur in skeletally mature patient. Wide zone of transition is appreciated, with soft tissue extension, with periosteal reaction seen and abnormal osteoid formation.
MRI – Altered Signal intensity lesion in right distal femur with cortical destruction and soft tissue extension.

Biopsy
Core needle Biopsy was done
HPE s/o Osteosarcoma

Staging investigation
HRCT Chest – no abnormality detected
Bone Scan – No abnormality detected

Summary and planning
Now this is case of osteosarcoma distal femur without metastasis.
Proper counselling was done regarding gravity of disease and planning and success rate. Pros and cons of treatment was discussed with patient and family.

Neo Adjuvant Chemo > Surgery > Adjuvant Chemotherapy was planned

Neo Adjuvant Chemotherapy – IAP protocol was started, in which Ifosfamide, Adriamycin & Cisplatin was given and 3 cycle was given preoperatively.

Post chemo swelling & pain was reduced and restaging was done and still non metastatic.

Surgery – Wide resection of distal femur and reconstruction with endoprosthesis was done.

Function of limb After Surgery

Adjuvant Chemotherapy was given

Follow-up is going on

Till Now 1 years of follow up Patient is doing fine
